SAINT VENOM MEDICAL POLICY

Client Form Policy: Why We Require Health Information

At SAINT VENOM your safety and well-being are our top priority. Before providing any nail services, we require all clients to complete a confidential client intake form.

This is not just paperwork—it’s an essential part of how we keep you safe and deliver the best possible care. Certain health conditions and medications can affect how your skin and nails react to the products and equipment we use, especially UV lamps and certain nail products.

Here’s why we ask for your health and medication information:

Medications and UV Sensitivity

Some medications increase your sensitivity to UV light, which we use in services like gel manicures. Increased sensitivity can lead to skin irritation, burns, or nail conditions such as onycholysis (when the nail separates from the nail bed).

Medications known to cause UV sensitivity include:
• Antibiotics
• Some antibiotics (like doxycycline, ciprofloxacin, and sulfonamides) can trigger phototoxic reactions when exposed to UV light, potentially leading to skin damage or changes in nail health.
• Tetracycline antibiotics, although less common, can cause nail discoloration.

• NSAIDs (Pain Relievers)
• Medications like ibuprofen, naproxen, and diclofenac can increase your risk of sunburn or blistering under UV exposure.
• Diclofenac is also linked to onycholysis, where the nail can lift from the nail bed.
• Acne Medications
• Treatments such as isotretinoin (Accutane) and tretinoin (Retin-A) can make skin and nails more sensitive to UV light, increasing the risk of burns or irritation.
• Antifungal Medications
• Azole antifungals, like ketoconazole, can cause photoallergic reactions when exposed to UV light.
• Other Medications
• Thiazide diuretics (like hydrochlorothiazide) used for blood pressure can increase photosensitivity.

If you’re taking any of these or similar medications, it’s important to let us know so we can adjust services to protect your health.

Diabetes and Nail Care

People living with diabetes require special attention during nail services.

Why is nail care so important for people with diabetes?
• Reduced Blood Circulation
• Diabetes can slow healing and make infections more serious.
• Nerve Damage (Neuropathy)
• Loss of feeling in the feet can make it hard to detect cuts, blisters, or nail injuries.
• Higher Infection Risk
• Even small wounds can lead to serious infections, ulcers, or complications.

Safe nail care tips for clients with diabetes:

✅ Inspect your feet and nails regularly for redness, swelling, or infection.
✅ Trim nails straight across without cutting into corners.
✅ Keep feet moisturized (but avoid lotion between the toes).
✅ Wear properly fitting shoes to protect feet.
✅ Maintain good blood sugar control for overall health.
✅ Seek professional help if you have thick, yellowed nails or any pain or infection.

Avoid:

🚫 Walking barefoot—even indoors.
🚫 Cutting or pushing back cuticles, which increases infection risk.
🚫 Attempting to remove corns or calluses yourself.
🚫 Ignoring signs of pain, infection, or changes in your feet or nails.

If you have diabetes, please let us know on your intake form so we can provide the safest and most appropriate care for you.

Your Privacy Matters

All information you share with us is kept confidential. It helps us customize services, avoid potential reactions, and keep you safe during your visit.

Saint Venom is not responsible for ANY medical bills due to you not listening and taking proper care of your nails as instructed. NAIL CARE IS ON THE WEBSITE UNDER NAIL CARE!

If you have a fungal infection or anything like that, Saint Venom has the right to refuse service if we deem that the nail is not ok to work on. We might ask you to reschedule.

SAINT VENOM SALON PROPERTY DAMAGE LIABILITY

✅ Client Responsibility for Damage:
You understand that you financially responsible for the cost of repair or replacement if you accidentally or intentionally damage any property belonging to SAINT VENOM NAILS during your visit.

✅ Items Covered Include (but are not limited to):
• Nail polishes and gels
• Nail tools and equipment
• Furniture or fixtures
• Laptops, iPads, and other electronics
• Decor items or displays
• Any other salon property

✅ Assessment of Damage:
You understand that SAINT VENOM will assess the value of any damaged items and will provide you with an invoice for repair or replacement costs, which you agree to pay promptly.

SAINT VENOM PUNCTUALITY AND CANCELLATION POLICY
At SAINT VENOM, we value your time and the time of our staff and other clients. To ensure the best experience for everyone, the following policies apply:

✅ Respect and Timeliness:
You agree to arrive on time for my scheduled appointment. Being punctual helps ensure that services run smoothly and fairly for all clients.

✅ Grace Period:
You understand there is a 10-minute grace period for appointments.
• If you arrive more than 10 minutes late, your appointment may be canceled and my deposit forfeited at the discretion of SAINT VENOM.

✅ Cancellation Policy:
You understand that:
• Appointments must be canceled or rescheduled at least 24 hours in advance of the scheduled time.
• Cancellations with less than 24 hours’ notice or no-shows may result in a charge or loss of deposit. If you reschedule and don't show up more than 2 times you will be fired.

✅ Courtesy and Communication:
You agree to communicate promptly if you are running late or need to cancel my appointment. You understand that respect and courtesy help maintain a positive salon environment for everyone.

SAINT VENOM CONSULTATION AND PRICING
Consultation and Pricing Acknowledgment

✅ Consultation Completed:
You confirm that SAINT VENOM will conduct a consultation with me regarding my requested nail services and that nail services are NON REFUNDABLE under any circumstance just like the deposit.

✅ Pricing Understood:
You understand and agree to the price quoted during my consultation for the services I have chosen.

✅ Deposit Required:
You agree to pay a $30 non-refundable deposit to secure my appointment. You understand this deposit will be applied toward my total service cost.

✅ Payment Policy:
You understand that full payment for services is due before the service is performed or after tips have been applied UNLESS dictated different from the Nail Tech. Cash, Cash App, Zelle, Apple Pay, Square Payments, PayPal.

✅ Additional Charges for Extras:
You understand that if you request any additional designs or enhancements not originally discussed during my consultation—including but not limited to charms, rhinestones, chrome, nail art, or specialty finishes—these will incur additional charges, which will be added to your total at the time of your appointment.

✅ Changes to Services:
Your understand that changes to my service choice on the day of my appointment may result in price adjustments or changes in appointment time.

SAINT VENOM CHILD POLICY AND SALON ENVIRONMENT
At SAINT VENOM, we strive to create a calm, relaxing, and safe environment for all clients and staff.

✅ Adult-Focused Environment:
SAINT VENOM is not a child-friendly environment. Our salon space is designed to ensure clients can relax and receive services without distraction.

✅ Guests:
We prefer clients arrive alone for their appointments whenever possible. However, we understand that sometimes a guest may need to accompany you due to anxiety or you just want a friend there—That is perfectly fine. If you bring someone with you, they must be an adult and not a child.

✅ Babies Are Welcome:
We understand that parents of infants often have unique needs. If you have a baby and need to bring them with you, you are welcome to do so, because everyone deserves to feel beautiful.

✅ Children’s Services Policy:
• SAINT VENOM does not perform nail services on children 17 years old or younger.
• The only exception is if a parent or legal guardian is present and has completed a consultation for the child’s service.
• All services for minors must be approved and scheduled in advance.

SAINT VENOM DOES NOT DO GEL POLISH OR ENHANCEMENTS ON KIDS YOUNGER THAN 18 YEARS OF AGE WITHOUT AN ADULT CONSENT.

SAINT VENOM RIGHT TO REFUSE SERVICE.

At SAINT VENOM, our goal is to provide exceptional nail services in a professional, safe, and respectful environment for both clients and staff.

✅ Right to Refuse Service:
SAINT VENOM reserves the right to refuse service to any client at any time, including during a service for any reason and or if it is determined that continuing service is not in the best interest of the client, staff, or salon environment.

✅ Reasons for Refusal May Include (but are not limited to):
• Health or medical concerns that make the service unsafe
• Open wounds, infections, or contagious conditions
• Disruptive, inappropriate, or abusive behavior
• Failure to follow salon policies
• Excessive lateness or no-show history
• Client requests that are unsafe, unrealistic, or violate salon standards
• Any other situation where the technician or salon feels uncomfortable or unsafe continuing the service

✅ Service Interruption:
If a service must be stopped for any reason, the client may be responsible for payment for work already performed up to that point, at the discretion of SAINT VENOM.

SAINT VENOM PHOTO, VIDEO AND SURVEILLANCE

At SAINT VENOM, we strive to showcase our artistry and ensure the safety of our clients, staff, and property.

✅ Photos and Videos of Services:
You understand that photos and videos may be taken of your nails, hands, or feet during or after your service for:
• Social media posts
• Marketing materials
• Website content
• Portfolio, educational purposes and safety.

You give my permission for SAINT VENOM to use these images or videos without compensation. You understand your or identifying features will not be shown without your separate consent.

✅ Salon Security Cameras:
You understand that SAINT VENOM has a security camera operating in the salon for the protection and safety of clients, staff, and salon property. This camera records general activity within the studio. It can be used in legal actions in necessary.
